Evidence-Based Practice Portfolio

The goal of this guide is to provide nurses and others a resource to understand and carryout the steps of performing a EBP project.


Please take this survey before starting your EBP(evidence-based practice) project. The purpose of this survey is to gauge your thoughts and feelings as they pertain to EBP and carrying out a project. Your response will be used to help evaluate how useful this resource is when it comes to carrying out a project and how one feels about evidence-based practice.


This online portfolio is a resource that will provide information and resources at each step in the EBP process. Some things to keep in mind as you start your project and utilize the portfolio.

  • It Takes a Team to Complete an EBP Project

Be sure that throughout the process of carrying out your project that you utilize your mentor and other experts within the organization.  In order to successfully carry out an EBP project, it takes a team, as each member of a team bring different skills and knowledge that will help ensure the EBP project will be successful.  These people may also be potential team members that could assist with the exploration of the literature, as well as play a key role in getting the project implemented on a unit. 

It is key that if you have never carried out an EBP project that you identify a mentor

  • Follow the Tabs to the Left 

The way to use this resource is to follow the tabs to the left.  The tabs follow the "Inquiry Process Diagram" and will provide you with resources and/or information you will need at each step in the process.  Notice that at the beginning of each section there are links to the needed Johns Hopkins tools and other forms you will need to properly document your project.   

  • Be Sure to Document What You Are Doing

It is important that you utilize the Johns Hopkins Tools and other documents provided at each step in the process to keep a record of what you and the team are doing.  This documentation is key when it comes to meeting with your mentor and ensuring the time is well spent, as well as enabling others to be able to evaluate the quality of your work.  These documents and the way articles are analyzed could be key in getting buy-in from people in other disciplines, as well when you decide to disseminate the findings of your work at a conference or in a paper.

  • Stopping Points Help to Ensure the Quality of the Project

There are stopping points are various points in the EBP process as you proceed through the portfolio.  These are intended to be times when one should meet with their mentor and discuss the progress being made as well as any problems or issues.  It is also during this meeting that any documentation that should have been completed is reviewed.  The intention of this review is to make sure that it is filled out correctly and that the project is moving forward as it should.  A project should not move forward until the mentor has signed off at each stopping point.   

  • Documents Uploaded into Web Submittal

Throughout the portfolio, there will be points where you will be asked to submit your documents via a web submittal process.  The purpose of this process is to enable the organization to be aware of all EBP projects that are being carried out, as well as at what point each is in the process.   It also serves as document repository so that all the documents that are needed to properly evaluate and carry out a project are preserved. 


  • After you submit a web submittal you will receive an email with the submittal number and a link to your project. Do not delete this email to your project. You will be working off this submittal throughout your project. 
  • Ask for Help Anytime You Have a Question

If you ever feel that you are at a point where you are stuck or are unsure what the next is or should be, do not hesitate to reach out to your mentor or others in the organization.  Any effort to improve care for patients or the healthcare environment is important and there are many in the organization who can and will help if asked.
