Enhanced Recovery After Surgery (ERAS) is well established in fields like colorectal surgery and aims to reduce postoperative complications and speed recovery through evidence-based perioperative care. This review presents consensus recommendations for ERAS protocols in thoracic surgery, mainly lung resections. A systematic review of high-quality studies was conducted, with lower-level evidence used only when necessary. Recommendations were developed by the ERAS Society and the European Society for Thoracic Surgery, covering 45 items across preadmission, intraoperative, and postoperative care. Key elements include preoperative counseling, smoking cessation, prehabilitation, carbohydrate loading, regional anesthesia, opioid-sparing analgesia, minimally invasive surgery, and early mobilization. Most recommendations are supported by strong evidence, suggesting that a structured ERAS pathway can significantly improve surgical outcomes.
