The following sources have places where you can download data relevant to health and disease.

Behavioral Risk Factors Surveillance System (BRFSS) Survey Data and Documentation
Documentation and download landing page for a large ongoing telephone health survey system from the CDC, tracking health conditions and risk behaviors in the U.S. yearly since 1984, including data files going back to the 1980s and documentation of changes in the survey.
ICPSR (Inter-university Consortium for Political and Social Research)
Even though the name suggests political science, ICPSR includes datasets from many different types of surveys, some of which address health. Use ICPSR to locate surveys with health-related questions and to download data.
National Center for Health Statistics Public-Use Data Files and Documentation
